Insurance
I am currently credentialed with Cigna insurance, and work with several EAP programs. It is your responsibility to be sure that you get the proper authorization needed according to your insurance plan. If your insurance requires pre-authorization, you need to make sure you have contacted your insurance company to obtain this information. Please bring the authorization number to your first session if required. You are responsible for payment if a proper referral is not obtained. Insurance companies require a mental health diagnosis. Please discuss this with me if you have concerns about having a mental health diagnosis on your medical record.
If I do not accept your insurance, I am happy to provide you with a monthly billing statement for you to submit to your insurance company. However, in order for you to be reimbursed by your insurance company, many require you to receive a diagnosis from your therapist. If you are concerned about having a mental health diagnosis on your medical record, please discuss this with me so we can talk about your options.
Accepted Payment Methods
I currently accept cash, checks and major credit cards. Payment is due at the time of appointment.
Cancellation Policy
Please note that I have a 24 hour cancellation policy. In the event you need to cancel an appointment, please provide notice within 24 hours of your scheduled appointment time. If sufficient notice is not provided on time or at all, you will be charged my normal session fee.
